2025-2026 HIGH SCHOOL AND MIDDLE SCHOOL SPRING SPORTS REGISTRATION INSTRUCTIONS
THE FAMILY ID ONLINE REGISTRATIONS FOR 2025-2026 SPRING HIGH SCHOOL AND MIDDLE SCHOOL SPORTS ARE OPEN!
Southern Lehigh is excited to announce that we are now offering the convenience of online registration for Southern Lehigh Athletic Programs through our partner company, Family ID. Through your registration on Family ID, you are completing the entire PIAA packet, and agreeing to the Code of Conduct and the Parents' Pledge. A Family ID registration must be completed for the Fall, Winter, and Spring seasons.

REQUIREMENTS FOR SPORT PARTICIPATION
Family ID Online Registration, PIAA Physical (PIAA Section 5 form and PIAA Section 6 form - see links below), Impact Concussion Test, Pay to Participate Fee.
PIAA Recertification form (PIAA Section 7 - see link below) — applicable to student athletes who played a 2025-2026 fall or winter sport or whose physical date is older than 6 weeks from the start of winter sports.
Physicals are valid if given on or after May 1, 2025 and shall be effective, regardless of when performed during a school year, until the latter of the next April 30th or the conclusion of the spring sports season.
If you are in need of a PIAA physical for the 2025-2026 spring sport season, you will need to obtain a physical from your family physician or visit a St. Luke’s Care Now Center for a $25.00 fee (fee subject to change - indicate that you are a Southern Lehigh Student-Athlete). Please ensure you have the correct PIAA physical forms: PIAA Section 5 Health History form and PIAA Section 6 Physician Physical form.
