FAMILY ID ONLINE REGISTRATIONS FOR 2026-2027 FALL HIGH SCHOOL AND MIDDLE SCHOOL SPORTS ARE NOW OPEN!
*NEW FOR 2026-2027* ALL PHYSICAL PAPERWORK IS SUBMITTED ONLINE IN FAMILY ID, THIS INCLUDES SECTIONS 5 & 6 OF THE PIAA PHYSICAL. SECTIONS 5 & 6 MUST BE SCANNED & SUBMITTED ONLINE IN FAMILY ID.
A Family ID registration must be completed for the Fall, Winter, and Spring seasons.

REQUIREMENTS FOR SPORT PARTICIPATION
Family ID Online Registration, PIAA Physical (PIAA Section 5 form and PIAA Section 6 form - see links below), Impact Concussion Test, Pay to Participate Fee.
FREE PIAA FALL ATHLETIC PHYSICALS – Wednesday, June 3rd in the Southern Lehigh High School Cafeteria from 5pm-8pm.
Click on the link below to sign up for a Free FALL Sports Physical. Please bring to the physical a completed Health History Section 5 form and a blank Physician Physical Section 6 form that are found on our website. Physical participants are to enter through the Athletic doors (door #7) across from the Tennis Courts. Please wear glasses/contact lenses (if required), athletic clothing, and sneakers. Note that a sport physical for the 2026-2027 school year cannot occur before May 1, 2026.

If you are in need of a PIAA physical for the 2026-2027 FALL sport season, you will need to obtain a physical from your family physician or visit a St. Luke’s Care Now Center for a $25.00 fee (fee subject to change - indicate that you are a Southern Lehigh Student-Athlete). Please ensure you have the correct PIAA physical forms: PIAA Section 5 Health History form and PIAA Section 6 Physician Physical form. Completed Sections 5 & 6 must be scanned and submitted online via the Family ID portal.

ATHLETIC IMPACT CONCUSSION TESTING
June 17, June 24, July 15, July 22, and August 5. 9:00am-10:00am in the HS Cafeteria. Each student should bring a laptop/chromebook. The athletic training staff will have a few extra if needed. There will be NO REMOTE TESTING!
This MUST be taken by all student-athletes in grades: 7, 8, 9, 10, 11, & 12.

HS FOOTBALL/ HS GOLF- July 30th
ALL OTHER FALL SPORTS- August 6th
*If these due dates are not met, the student-athlete may be held out of their sport on the first day of tryouts.*
Participation fees if paying by check (payable to "SLSD") or exact cash can be turned into the HS main lobby (box marked "SL Athletics"). Pay to participate fees will be deposited or refunded after final rosters have been announced. This process may take several weeks.
