    2024-2025 School Year ONLY:

    Important Information: Children are eligible for admission to kindergarten if they have attained the age of five (5) years on or before September 1st, 2024 (birthdate would then be on or before September 1st, 2019).

    The following documents are required upon registration. MAKE SURE YOU HAVE THESE DOCUMENTS SAVED AND READY FOR UPLOAD PRIOR TO MOVING TO ON-LINE REGISTRATION.  

    1. Birth Certificate (preferred), additional acceptable documents are noted below:
      1.  Notarized copy of a birth certificate, baptismal certificate, copy of the record of baptism – notarized or duly certified and showing the date of birth, a valid passport, or a prior school record indicating the date of birth.
    2. TWO proofs of residence:
      1. Acceptable documentation includes: 1) Rental/Lease or purchase agreement of a permanent SLSD residence 2) Payment receipt of appropriate state and local taxes with SLSD address 3) Pennsylvania driver’s license and/or automobile registration with SLSD address 4) Current Utility bills (water, sewer, heat, oil electricity) with SLSD address (no cell phone bill) 5) Pennsylvania identification card 6) Current pay stub with address of employer and employee 7) Current medical, state, or federal benefits document showing SLSD
    3. Immunization Records
    4. Physical Examination Form Grades K, 6 & 11 
    5. Dental Examination Form Grades K, 3 & 7
    6. IEP (if applicable)
    7. Custody agreement (if applicable)

    Online registration 

      1. Select click the following link: SLSD 2024-2025 Registration 
      2. Please select Start New Registration then click next.
      3. The system will now ask for Parent/Guardian information including YOUR date of birth. Please make sure you input a valid email address. This is required by the system in order to receive a link to begin your application. 
      4. Once you hit "begin" to launch the application, you will receive your application number.  Please make note of your application number. You will need this number to: start and return to the application at a later date or request assistance regarding your student's application.
      5. Any field with a * is a required field, the system will not permit parents/guardians to move forward until all required fields are completed. When in the system you will see NEXT buttons and Save and Continue buttons, please make sure you go through all fields under NEXT before hitting Save and Continue. 
      6. If you are having difficulties with your application, please make sure you have your application number ready when contacting the Registration office.
