For STUDENT SUPPORT & OPERATIONS EMPLOYEES, there is a total of 26 forms that new employees will need to complete and submit before they begin service with the Southern Lehigh SD. Each form (except for signature/ consent forms) is a fillable Adobe Acrobat (*.pdf) file. Thus, you are strongly encouraged to complete each form on your home desktop or laptop, which can be either a Windows or Mac computer. Please read each form carefully.
Once you have completed ALL forms, you can either email them to or make an appointment with the Office's HR Generalist, Debra Miceli (micelid@slsd.org). Prior to your appointment, please print ALL forms including those for which you might need clarification and/or assistance. Our office location is 5775 MAIN STREET | CENTER VALLEY, PA 18034.
Please do not mail us your completed forms; there is a risk that the form will either be lost in the mail or not arrive in time for the start of your service. Lastly, if you have any questions about one or more forms, technical or substantive, please contact our office at hr@slsd.org.

[1] OFFICIAL COLLEGE OR UNIVERSITY TRANSCRIPT (OPTIONAL)
If you have obtained an Associate's degree or higher, you should provide an official college or university transcript to include with your employment record. An official transcript is one that is provided by the institution's Office of the Registrar and must be ordered directly through the institution's website. These documents are provided either electronically (via email) or hardcopy (via postal mail). Electronic official transcripts are offered as secure Adobe Acrobat (*.pdf) files whereas hardcopy official transcripts are issued on institutional letterhead such as blue tamper-proof security paper with a raised seal of the institution and the signature of an institution's administrator.
If you choose to provide us with a transcript, please do not provide us with an unofficial transcript. These are generally accessible via your institution's online student portal and are designed for you (and not a prospective employer) to review your academic record. They will not have the security features associated with an official transcript.
[2] INDUSTRY OR TRADE CERTIFICATION (OPTIONAL)
Many professional associations offer their members industry-wide recognized credentials that certify their skillsets in one or more areas of expertise. Some examples include the (a) North American Technician Excellence (NATE), which offers certifications in various HVAC specialties like gas furnace, air conditioning, and heat pumps or (b) the International Brotherhood of Electrical Workers (IBEW), which offers certifications for inside wiremen, outside lineworkers, and other electrical specialties. Instructional Assistants can also provide evidence of their competencies through assessment results such as the Educational Testing Services's ParaPro Assessment; more information about this exam can be found by clicking HERE.
[3] PROFESSIONAL LPN/ RN LICENSURE (REQUIRED FOR LICENSED PRACTICAL OR REGISTERED NURSE ONLY)
If you are hired as a Licensed Practical Nurse (LPN) or a Registered Nurse (RN), you will need to provide your official Licensed Practical Nurse (LPN) or RN license as part of your onboarding paperwork. You can obtain a copy of your license via the State Board of Nursing by clicking HERE.
[4] BLS CPR (REQUIRED FOR LICENSED PRACTICAL OR REGISTERED NURSE ONLY)
If you are hired as a Licensed Practical Nurse (LPN) or Registered Nurses (RN), you will need to provide your (BLS CPR), as part of your onboarding paperwork. You can obtain a copy of your license(s) from the granting agency such as American Red Cross (ARC) or the American Heart Association (AHA).
If you need to obtain (or renew) your CPR licensure, you can do so with one of our two AHA-certified CSNs (E. Harwick and N. Castetter). Candidates will need to successfully pass a hybrid course in which they must first complete an online portion and then schedule the hands-on testing portion with either AHA-certified CSN. Please note that completion of the online portion is a prerequisite for testing.
