For COACHES & ANCILLARY (EXTRACURRICULAR) EMPLOYEES, there is a total of 21 forms that new employees will need to complete and submit before they begin service with the Southern Lehigh SD. Each form (except for signature/ consent forms) is a fillable Adobe Acrobat (*.pdf) file. Thus, you are strongly encouraged to complete each form on your home desktop or laptop, which can be either a Windows or Mac computer. Please read each form carefully.
Once you have completed ALL forms, you can either email them to or make an appointment with the Office's HR Generalist, Debra Miceli (micelid@slsd.org). Prior to your appointment, please print ALL forms including those for which you might need clarification and/or assistance. Our office location is 5775 MAIN STREET | CENTER VALLEY, PA 18034.
Please do not mail us your completed forms; there is a risk that the form will either be lost in the mail or not arrive in time for the start of your service. Lastly, if you have any questions about one or more forms, technical or substantive, please contact our office at hr@slsd.org.

[1] INDUSTRY OR ATHLETICS CERTIFICATION (OPTIONAL)
Many professional associations offer their members industry-wide recognized credentials that certify their skillset in one or more areas of expertise. Some examples include the (a) National Association of Sport and Physical Education (NASPE), which offers certifications for various sports, including aquatics and physical education or (b) Screen Actors Guild-American Federation of Television and Radio Artists (SAG-AFTRA), which recognizes a performer's experience and contributions to the performing arts.
